Lindsey Fulford has been a member of our team for several years and has recently transitioned to the Care Management Coordinator role. Care Management is a program to help you get the care and support you need between office visits. Lindsey, your Personal Care Coordinator is there to listen, understand what you need and assist you with various challenges regarding your health and wellness. She will assist you with any medication questions, identify community resources that can help you, help you identify and reach your health goals, and keep your nurse practitioner up to date on how you are doing.
